Our national team of clinical pharmacists and nurses collaborate across all levels of primary care, providing additional clinical capacity, mentorship, education, and proactive patient reviews to enhance patient access.
As an NHS business partner, the Interface team help primary care improve health outcomes, reduce health inequalities, and deliver targeted resources for proactive care.
Working with around 50% of all GP practices and PCNs, our clinicians focus on key therapy areas such as asthma, COPD, CVD, diabetes, and lipid management. In 2025, Interface pharmacists reviewed over 720,000 patients, significantly improving patient outcomes.
For example, Interface pharmacists initiated cardioprotective medication for over 8,500 diabetes patients, preventing 121 heart attacks. Proactive asthma reviews prevented over 16,500 asthma attacks and freed up more than 6,500 additional nurse appointments.
