The Population View on Abtrace's Proactive Monitoring Tool lists all patients of the GP practice who have requirements according to the monitoring guidelines. This can be sorted or filtered and exported to CSV. By clicking a patient within the table, opens the Patient View. The HCP can send direct messages, either by email or text, to a specific patients or a group of patients using this view.
