This talk will discuss the development and implementation of an integrated virtual CKD program (LUCID), from conceptualisation, through operationalisation to commissioning. It will discuss the impact of the program on care systems and the steps taken to demonstrate impact and efficacy that supported funding. It will also discuss the integration new technologies and innovations to improve patient self-management, staff training and precision case finding.
