Neighbourhood matters: tackling health inequalities in the east of Barking & Dagenham

Over the past three years, East One Primary Care Network has brought together the voices and lived experiences of local residents with GP Federation supported analytical population health data to design interventions that truly reflect community need. Through neighbourhood partnerships with charities, local businesses, the local library and learning centre, the local authority, and residents themselves, the East One PCN Health Inequalities team has delivered targeted pop up clinics focused on women’s health, men’s health, over 65s health, children’s asthma care, mental wellbeing, and hypertension, all aligned with the Core20PLUS5 approach to tackling health inequalities.
This work also includes Hypertension Outreach, a Children and Young People’s Patient Participation Group (Youth PPG), and Winter Coat Drives supporting families facing hardship. The Children’s and Asthma Drop In Clinic received national recognition, winning the HSJ Partnerships Awards 2025 for Best Primary and Community Care Project of the Year.
