Pharmacy first, everywhere: harnessing PCN pharmacy teams to turn medicines into neighbourhood impact
PCNs are now home to some of the largest changes in pharmacy workforce and medicines optimisation in a generation, with pharmacists working across general practice, community pharmacy and wider system teams. In this session, we will discuss examples of how integrated PCN pharmacy teams have reduced GP workload, improved medicines safety and delivered better outcomes for diverse populations – from frailty and care homes to underserved communities. We will explore what makes integration succeed in practice: clear roles, cross sector collaboration, robust supervision and a shared focus on quality and safety. Delegates will leave with a practical 90 day framework resource tool to strengthen pharmacy integration in their own PCN or neighbourhood, whatever their clinical priorities.
By the end of the session, delegates will be able to:
- Describe at least three real world models where integrated PCN pharmacy teams (across general practice, community and other sectors) have improved access, safety, workload and patient experience.
- Identify the organisational, cultural and governance factors that enable successful integration of PCN pharmacists and wider pharmacy workforce into multidisciplinary teams.
- Outline practical approaches to supervision, support and career development for PCN pharmacists and independent prescribers that work across multiple practices and sectors.
- Draft a 90 day, cross sector pharmacy led improvement plan for their PCN or neighbourhood, aligned with ICB medicines optimisation priorities and CQC expectations, regardless of clinical speciality focus.
