This session explores how we in Brent in NWL have incorporated our ARRS PCN pharmacists onto Fellowship roles where they have been able to successfully create PCN led diabetes clinics reducing waiting lists in secondary care. It will demonstrate how utilising and upskilling our current workforce has helped to improve patient care, increase skillsets, reduce waiting lists and has enabled teams in acute trusts to manage more complex patients.
We will be sharing practical insights, clinical experience, and actionable strategies that clinicians can apply directly within their own practice.
- Understand current challenges and opportunities in delivering effective, patient-centred care within primary care settings
- Apply practical, evidence-based strategies to improve clinical outcomes and service efficiency
- Recognise the value of multidisciplinary and pharmacy-led approaches in managing long-term conditions
- Identify actionable changes that can be implemented immediately within their own practice or organisation to improve patient access and clinical outcomes
- Reflect on how service innovation and leadership can drive sustainable improvements in patient care
