In this session, we walk through common opportunities and challenges faced in the delivery of digital first clinical reviews in primary care and how a “digital-first” approach can deliver efficiencies, increase accessibility and engagement, whilst improving the patient and clinician experience and achieving successful targets. Our team will present a case study using asthma reviews and how we can partner to scale the digital first solution across multiple clinical conditions.

PCNs will be invited to join a national initiative to co-design deploy a best practice pathway for digital first long-term condition management on the iPLATO+Huma Platform.
