An overview of how Brookside Group Practice use segmented digital triage to improve access and continuity for patients. This case study includes organisation enablers such as organisation structure and capabilities, Patient Health Teams model of care and ARRS workforce. We will also discuss the different digital solutions we use such as Anima online consultation software and Segmentation created by Connected Care. Finally, we will talk through the outcomes for us as a Practice and next steps for tackling demand and developing the systems and processes we use further.
